Important reminder for first time patients:

Please arrive to the office 15 - 20 minutes before your scheduled appointment.

If possible download, print, and fill out the following documents: New patient forms. This will expedite your initial check-in. If you don’t have access to a computer or printer, the front desk will provide you with the forms when you arrive.

Bring your insurance card and an official picture ID (driver’s license or passport, for example) We will need to make a copy of both.

If you have had an X-Ray or MRI done, please bring a copy of the CD and the written radiology report with you.

Medications list, if any

If you have had surgery elsewhere, please bring a copy of your operative report.

Hours of Operation
Offices are open from Monday to Friday, from 8am to 5pm. An answering service is on call before 8am and after 5pm, and during lunch from 12pm-1pm.
How do I make an appointment?
You can make an appointment with us by:
1. Call us at 404-299-1700
2. Emailing us at frontdesk.uoc@gmail.com (Please do not include sensitive medical information on your emails). Please include your full name and best contact number. Someone will contact you within 24 hours to schedule your appointment.
3. You can also reach us by clicking HERE and filling out a online appointment request.
What to bring to your first appointment/consultation?
To better serve you and to expedite your check-in process please make sure you bring the following to your first visit:

Cancelation Policy
We understand that sometimes schedules change and that you may need to cancel or re-schedule your appointment. You can cancel your appointment without charge with a 24hr notice. Any appointments canceled with less than 24 hrs or No-Shows will incur in a fee of $25-$100 (depending on the type of appointment)
Late Arrival Policy
In order to mantain work flow and see patients in a timely fasion, if you are more than 20 minutes late, you are subject to being re-scheduled. If time and space is available we will try to work you in but can't guaratee the appointment time.
